Please fill out this form at least one week prior to the field trip date. Cheques will be ready no later than 5 days after the field trip form is received. Pick up cheques in the PAC drawer in the Staff Room.

 

Reminder: given Gaming Fund restrictions, cheques must be written to a third party. Teachers cannot be personally reimbursed for field trip monies spent. Please leave documentation that supports cheque requests in the PAC drawer marked "FAO of Willows Treasurer".
